At Mikolich Orthodontics, we take pleasure in contributing to our community and expressing gratitude to our patients. This inspired the creation of our patient referral initiative, Share a Smile. This program is our expression of thanks for bringing new patients into our practice!
REFERRAL GUIDELINES & GRATITUDE
If the friend or family member you refer begins their treatment within six months of their consultation, you’ll earn a $150 credit applied to your existing treatment plan! Please take note of the following:
- A “Referral” pertains solely to new patients; existing patients or those who have previously visited our office are not eligible.
- Complete the Share a Smile referral form below before the referred patient attends their consultation.
- The referred patient must undergo the initial consultation, initial exam, and necessary X-rays.
- The $150 account credit will be applied to the patient’s balance and cannot be combined with any other offer.
- To be eligible for the $150 credit, the referred patient must initiate Invisalign, Phase 1, Phase 2, or Comprehensive orthodontic treatment, within six months from their consultation date.
